Participant Waiver, Release of Liability & Assumption of Risk

This is the waiver every player will physically sign at check-in on October 3, 2026. Online registration does not replace the physical signature.

I, the undersigned, wish to participate in the Bree's Fall Bash Kickball Tournament hosted by Bree's Legacy (The Breanna Leigh Chadwick Memorial Foundation, Inc.).

Assumption of Risk

I understand that kickball is a physical activity with inherent risks including slips, trips, falls, collisions, being struck by a ball, weather conditions, or overexertion. I knowingly and freely assume all such risks, whether known or unknown.

Release of Liability

In consideration of being allowed to participate, I release and hold harmless Bree's Legacy, its directors, officers, volunteers, staff, sponsors, vendors, and the City of Canton from any and all liability for injury, illness, disability, death, or property damage arising out of participation, except in cases of gross negligence or willful misconduct.

Medical Consent

I certify that I am physically fit and able to participate. I consent to necessary first aid and emergency treatment, and I accept responsibility for any medical expenses incurred.

Conduct

I agree to follow all tournament rules and staff instructions. I understand that alcohol, illegal drugs, and unsportsmanlike conduct are strictly prohibited.

Media Release

I grant Bree's Legacy permission to use photographs, video, and recordings of me from this event for promotional and fundraising purposes without compensation.

Acknowledgment

I have read and fully understand this waiver and release. I sign it voluntarily and acknowledge it will be binding on me, my heirs, and assigns.

Every player must sign this waiver before their first game. If the participant is under 18, a parent or legal guardian must sign on their behalf. Physical waivers will be collected on event day before the player is allowed to participate.
